To the contact people of all Cochrane entities -

This email is a reminder to entities that are yet to allocate their sponsored entity registration. 

As announced at the end of last year, the Steering Group has agreed to use the Collaboration’s core funds to pay for the registration for one person from each Cochrane entity for the Colloquium in Melbourne.  The early registration deadline of 15 July 2005 is approaching and we would like to encourage all entities to allocate their place and ensure their nominee is registered by the this date.  Instructions are provided below. 

If you are unable to use the place, you may like to consider donating the registration to the stipend fund.

To allocate your sponsored entity registration, please send the name and email address of the recipient, together with the name of your entity, to Sue Brennan (sue.brennan@med.monash.edu.au).  We will create a master list of recipients to ensure there are no unauthorized claims.

Your designated person must then register for the Colloquium online at http://www.icms.com.au/colloquium/register/entity.asp

Please note that this registration form is different to the online form on the registration page of the Colloquium website and is only to be used for sponsored entity registrations.  Please do not share the URL with anyone else in your entity or use the form for other registrations.  For those unable to use the online system, a pdf version of the sponsored entity registration form is available from Sue Brennan.  All sponsored entity registrations will be checked against the master list of recipients.
Please note that sponsored entity registrants will be required to pay for their own accommodation, optional social events and any additional tickets to included social events.
