Educational interventions for adults to prevent readmission and complications following cardiovascular surgery

1. To assess the effects of individualized patient education interventions compared to standardized education on hospitalizations, health related quality of life, mortality, performance of specific health behaviours (for example smoking and physical activity), mood (depression/anxiety), and cognition during the post-hospital discharge recovery period following cardiovascular surgery.

2. To examine the effect of mode, dose, and timing of individualized patient education interventions on health related quality of life, hospitalizations, mortality, performance of specific health behaviours (for example smoking and physical activity), mood (depression/anxiety), and cognition during the post-hospital discharge recovery period following cardiovascular surgery.
